[dcmf] Re: Contributor License Agreements

It occurred to us that including scanned copies of signatures in the
repo might not be the best move ever.  Based on that, we are merely
including a single file listing the names and email of those who have
filled out the forms (and, if needed, the employer).  Management here
will be handling the documents, and we intend for them to follow
whichever organization remains/becomes the DCMF code maintainer in the
future.

If you would like to contribute code (or have already), please fill
out the form and get it back to us.  We would very much like to be
sure that this gets off to a good start.  If you have any concerns.
please contact me.
